Good Faith Estimate (GFE)
Fee for professional service is based upon years of experience, level of training, value and expertise.
A good faith estimate (GFE) will be provided during your intake session for therapy clients. We will collaboratively explore your unique treatment needs and course of recommended care. In today’s digital age, your privacy is our priority and we take this very serious as a professional practice. One of the ways we help safeguard your privacy is limiting third parties’ access to your mental health history and care as indicated by law, including non-disclosure of sensitive information to health insurance companies. Therefore, we are an out-of-network practice.

How to Request Out-of-Network Access to Insurance Benefits:
1. Since we are an out-of-network provider, upon request the Practice can provide you with a Superbill to submit directly to your insurer.
2. You have the option to sign up for a mental health benefit program known as Mentaya. This resource provides an instant benefits checker for clients. If interested, you can learn more by reviewing their company website: https://www.mentaya.com/patients
3. Contact your insurance company directly to inquire about your specific benefits. You may ask the customer service representative, “Does my plan include out of network benefits to receive outpatient mental health care?”.
4. Inquire if you have a deductible to meet before coverage is approved. You can ask the customer service employee, “Do I have a deductible to meet to receive out-of-network mental health services and if I do what is the remaining amount I need to pay before this health plan will reimburse me for anything I have paid out of pocket to receive care?”.
5. Ask, “How much will my plan reimburse for mental health care code 90837 with a licensed psychologist? If this information is not provided you can inquire, “What is the maximum permitted amount for mental health code 90837 with a licensed psychologist and what percentage of the maximum allowed amount that my plan will pay?”.
6. As a private pay Practice that respects your privacy, it is important for the client to coordinate care when you authorize third party involvement in your care.
